**SPECIAL ANNOUNCEMENT**

On February 1, 2016, we will become Fruita Family Medicine of the Grand Valley, under management of Community Hospital.

 

WHY ARE WE DOING THIS?

Changes in healthcare technology have resulted in new requirements from both private and government insurance companies. Insurance paperwork and electronic reporting have become increasingly more complex to manage, and it has become very difficult to run an independently-owned office while providing quality patient care.  We have been considering different options for the past couple years and have decided that we need to partner with a larger entity that can provide the support we need to continue to care for our patients.  We feel that Community Hospital is an excellent choice for this.

WHAT DOES THIS MEAN FOR YOU AS A PATIENT?

We do not foresee any significant changes in the day-to-day operations of the office or in the care of our patients.  We plan to continue to provide care for you in the same way that we have over the past decade.  You will still be able to choose which hospital or surgical center to go to if you need to be admitted or undergo a procedure, the laboratory where you would like to have tests done, and the facility where you would like to have x-rays and other studies performed.  You will have the same choice of consultants and specialists as you had previously. We will still accept all of our current insurance plans. Our phone number will stay the same.  Our office will stay at the same location.  All of our staff plans to stay.

Some things that you may notice changing are as follows:

   -Community Hospital will be handling billing and collections in the future. You will be receiving financial notices from them.

  -We will be referring you to Community Hospital for lab tests, x-rays, etc. unless you specifically request that they be done someplace else.

  -Our electronic health record (EHR) system will change to the one used by Community Hospital and Grand Valley Primary Care.  Our patient portal will also change, so MyHealthWare cannot be used for electronic communication after Friday, January 29, 2016. We thank you in advance for your patience while we transfer your files and re-register you for a new portal connection.

We will continue to provide in-hospital care for newborns at whichever hospital you choose to deliver your baby.  In-house hospitalists and medical residency teams will continue to manage our hospital care of adult and pediatric patients.  We will continue to collaborate directly with the hospital teams and provide outpatient follow-up care, as we have been doing successfully since 2010.

 As with all changes, there will be challenges.  Even so, we are very excited about this opportunity to partner with Community Hospital.  We feel that this will be an excellent relationship that will allow us to continue to provide high-quality care for our patients, as we have for years.   If you have questions or concerns, please let us know.
